To further standardize health examination practices and protect the health rights of examinees, the National Health Commission has organized the formulation of the Recommended Guidelines for Adult Health Examination Items (2025 Edition). These are hereby issued to you for reference and use by health administrative departments at all levels and relevant medical institutions in their adult health examination work.

Recommended Guidelines for Adult Health Examination Items (2025 Edition)

To guide health examination institutions in appropriately establishing health examination items, the Recommended Guidelines for Adult Health Examination Items (2025 Edition) (hereinafter referred to as the "Guidelines") are formulated.

I. Scope of Application

The "health examination" referred to in these Guidelines means the diagnostic behavior of conducting physical examinations on examinees through medical means and methods to understand their health status and detect early disease clues and health risks. It does not include occupational health examinations, employee health checkups, special physical examinations mandated by national regulations for purposes such as school enrollment, military enlistment, or marriage registration, health checkups provided under basic public health service programs, pre-pregnancy eugenic health checks, or screening and普查 (general surveys) for specific diseases.

These Guidelines apply to health examinations for the adult population aged 18 and above.

II. Main Content

(1) Health Examination Self-Assessment Questionnaire (Annex 1). This guides examinees to truthfully complete a self-assessment questionnaire to understand their basic health status, conduct a preliminary disease risk assessment, and help reasonably establish examination items. The questionnaire includes: basic personal information, past medical history and family history, lifestyle information, and mental health status.

(2) Basic Examination Items (Annex 2). These include physical examination, laboratory tests, and auxiliary examinations.

  • Physical Examination: Includes general examination and physical inspection.

    • General Examination: Height, weight, waist/hip circumference, blood pressure, pulse rate.

    • Physical Inspection: Internal medicine, surgery, ophthalmology, otorhinolaryngology (ENT), stomatology (dental/oral), and gynecology (for individuals with a history of sexual activity) examinations.

  • Laboratory Tests: Include routine tests, biochemical tests, thyroid function tests, molecular tests, and cytological tests.

    • Routine Tests: Complete blood count (CBC), urinalysis, stool routine + occult blood test.

    • Biochemical Tests: Liver function, kidney function, blood lipids, blood glucose, blood uric acid, blood homocysteine.

    • Thyroid Function Tests: Thyroid-stimulating hormone (TSH), free thyroid hormones.

    • Molecular Test: Primarily refers to high-risk HPV nucleic acid testing.

    • Cytological Test: Primarily refers to cervical cytology (Pap smear).

  • Auxiliary Examinations: Include electrocardiogram (ECG), radiological examinations, ultrasound examinations, and body composition analysis. For individuals aged 40 and above, pulmonary function testing and bone mineral density testing are added.

(3) Chronic Disease Risk Screening Items (Annex 3). Building upon the basic examination items and combined with the health examination self-assessment questionnaire, personalized in-depth examination items should be added for individuals identified as having risks for diseases such as cardiovascular and cerebrovascular diseases, common malignant tumors, chronic respiratory diseases, and diabetes. These additions should follow the corresponding chronic disease screening and early diagnosis/treatment protocols.

III. Key Considerations

(1) Health examination institutions may refer to the Adult Health Examination Workflow Chart (2025 Edition) (Annex 4) to conduct their work. When establishing health examination items, they may reasonably adjust the items based on the examinee's health service needs and the institution's own professional technical personnel structure and medical equipment configuration.

(2) Health examination institutions should use the Common Chronic Disease Risk Assessment Indicators (Annex 5) to screen populations at risk for chronic diseases and select appropriate screening items.

(3) Health examination institutions are encouraged to fully utilize information technology to support health examination and health management services.
